公告：感染症アウトブレイクの端緒情報報告について
I．状況の報告
以下5種類の異常事態を発見した場合、直ちに手がかりを提供されたい。
（一）周囲の人々や家禽、家畜などの動物に濃厚接触した後になって、発熱や咳などの症状を発症した患者、或いはその他の重篤な症状を発症或いは死に至った場合；
（二）周囲の人々が、マーモットなどのペスト宿主と接触した後に、発熱やリンパ節の腫れ、咳などのペストの疑いがある臨床症状を呈した； 周辺で発病した（死んでいる）マーモットやその他の病気（死）したペスト宿主の動物を確認した場合。
（三）ペスト発生率の高い国・地区から来た人や、屋外観光をした人、屋外作業・活動をした人がその後に発熱や発疹、脳炎、髄膜炎など感染症が疑われる症状が発現した場合；
（四）周囲の人々が多数短期間に発熱や咳、発疹、嘔吐、下痢などの感染症の疑いのある症状を発症した場合；
（五）近隣で多数の人々に原因不明の高熱、或いは急死した、或いは原因不明の病気や感染症の発生により突然死亡する。
二、報告ルート
感染症の発生に関する情報は、市、県（区）の疾病管理センターの24時間対応ホットラインで連絡可能だ。
三、報告のガイドライン
報告の際は、発生日時や詳細な場所、関係者の状況、症状の特徴、暴露歴及び本人の実名情報と連絡先

I. Reporting Situations
Please provide information immediately if you encounter any of the following five types of irregularities:
(I) A cluster of patients exhibiting symptoms such as fever and cough—or other severe symptoms or death—following close contact with poultry, livestock, or other animals;
(II) A cluster of patients exhibiting symptoms such as fever and cough—or other severe symptoms or death—following close contact with poultry, livestock, or other animals;
(III) If a person(s) who has arrived from a country or region with a high incidence of plague, or who has engaged in outdoor sightseeing, work, or activities, subsequently develops symptoms suggestive of an infectious disease—such as fever, rash, encephalitis, or meningitis;
(IV) Many patients in one’s vicinity exhibiting symptoms of suspected infectious diseases such as fever, cough, rash, vomiting, and diarrhea within a short period of time;
(v) When people around you experience sudden death from unexplained high fever, or when there is an unexplained illness or outbreak that may be infectious.
II. Reporting Channels
You may report leads regarding infectious disease outbreaks by calling the 24-hour duty lines of municipal or county (district) Centers for Disease Control and Prevention (CDCs).
III. Reporting Guidelines
When reporting, please provide accurate details regarding the time and specific location of the incident, the individuals involved, symptom characteristics, and exposure history, as well as your real name and contact number; ensuring the information is accurate and traceable will facilitate rapid verification and response.
- Handling of Tips
We will maintain strict confidentiality regarding any tips received; the information will be used solely for epidemic prevention and control purposes, and risk assessments, investigations, and follow-up actions will be carried out promptly.

关于公众报告传染病疫情线索的公告
一、报告情形
当您发现以下5类异常情况时，请第一时间提供线索：
（一）身边人群与家禽、家畜等动物密切接触后，出现较多发热、咳嗽等症状患者，或出现其他严重症状或死亡现象；
（二）身边人群与旱獭等鼠疫宿主动物接触后，出现发热、淋巴结肿大、咳嗽等疑似鼠疫临床症状；周边地区发现病（死）旱獭、其他病（死）鼠疫宿主动物死亡。
（三）身边人群从疫情高发国家/地区、野外旅游、野外工作/活动返回后，出现发热、出疹、脑炎脑膜炎等疑似传染病症状；
（四）身边人群短期内出现较多发热、咳嗽、出疹、呕吐、腹泻等疑似传染病症状的患者；
（五）身边人群出现原因不明的高热急死病人，或出现可能具有传染性的不明原因疾病或聚集性疫情。
二、报告途径
您可以通过拨打市、县（区）疾控中心24小时值班电话报告传染病疫情线索。
三、报告规范
报告时请如实提供线索发生时间、详细地点、涉及人员情况、症状特征、暴露史及本人实名信息与联系电话，
